You can use All Trails without a connection if you are a premium subscriber, which is around $30/year. They have a sale about twice a year that brings the price down to $15. This allows you to download many different types of maps to use offline.
If the trail is on Google Maps, you can download a custom map for the area and use it offline without a connection. It only downloads the map though, and not the aerial images.
Trailsoffroad.com has downloadable .gpx files. They don't have an app (as far as I know), but they are a great resource for trail information.

I have been using Gaia GPS for a little over a year and it seems really complete. Ability to take pics while tracking is nice, and the map I use most is USFS 2016 - if I see a trail, it's 99% of the time on the map (or vice-versa). Also, while there are plenty of free maps, if you want to pay, there are tons more.

I just recently started using GaiaGPS. You need connectivity to download maps but then works offline as long as you have all the maps downloaded ahead of time. For the better downloaded maps, they have subscription fees which are very reasonable and allow unlimited downloads.
I think it has more features on Apple phones/tablets than on Android. I have the Android version which is good but hard to tell if you have all the maps downloaded that you want/need for a given area.

All Trails and Gaia are both good, but personally I use ViewRanger. It has a decent selection of free basemaps, and I find it easier to upload and download GPX files than with the other apps.

I use Backcountry Navigator Pro but I'm not sure if it is available for iphones or not. Definitely the best I've tried, you can save imagery, topo, usfs maps, etc. Also lets you log tracks as your drive, hike, etc.
