Can anyone identify this lift?

Use the numbers on that sticker and type them into Google. That should bring something up.

Whatever it is it's a cheap lift given that those are fixed length control arms (garbage IMHO). The shocks are Bilstein 5100s.

I concur, Rough Country. @TheBoogieman should be able to confirm this just by looking at the paint quality.

It's the newer RC control arms, with rubber bushings, not the old poly bushings also.

The rubber bushings may help, but you really need something like a Johnny Joint at one end (if possible) of those control arms. Those arms are a fixed pipe and welded solid at both ends. That means they do not twist like the factory arms. Therefore the brackets on the axle and frame take a lot of force under articulation. Basically it is trying to rip the brackets off because the arms don't twist.
