Control arm bushing

Front axle, pass side upper CA at axle.
It seems this has happened to me before, again and now again.
Does anyone know why this bushing keeps blowing out to one side.
I think it is or was properly installed. Not too hard to do.
This is not the first time I’ve experienced this.

Bushing was brand new. Had a little death wobble I fixed with a tire balancing. Went wheeling (pretty heavy) Spent the day yesterday PM’ing everything and found this.
Last time I found this was when i pulled my Dana 30 for the 44 swap the “new” bushing in the 30 was i. The same condition.
