Does anybody have trouble with wheel spacers coming loose?

Does anybody have trouble with wheel spacers coming loose?
The rear drivers side came loose with less than 150 miles. Though there might be a trick to it I didnt know about.

The nuts came loose from the studs holding the adapter? If so, clean the stud threads then apply a couple drops of red loctite to the threads all of the studs. Tighten all of the nuts to 80 ft-lbs. Don't apply Loctite to the adapter's stud threads that hold the wheels on. Clean those threads and either tighten them to 90 ft-lbs dry or apply a dab of anteseize to them and tighten them to 80 ft-lbs.

If they were installed properly, they should never come loose.

I did all that, I cleaned and added more red loctite to it and forked it down again, the other 3 are fine
