I need help identifying this part and hose. The hose runs to it and the engine. The ends are dry rotted and it keeps popping off.

Just to show exactly what I’m talking about and where the hose connects…it’s on the driver side right in front of the brake booster
That is a vacuum line for the fuel vapor recovery system.
You should be able to purchase new rubber ends at the Auto Parts self help section and reuse the poly tubing....

It's #53013034AB (Hose-LDP To Intake Manifold); however, it's discontinued. Here's a previous thread to peruse (the discussion included alternate solutions):

That is the purge valve or vapor management valve. You can buy a piece of vacuum line and replace the plastic hose completely.
