After pulling my hair out over the correct wiring for a 5 terminal lighted toggle switch I finally figured it out. Hopefully the following will help others struggling.

To clarify somewhat. That is not a 5 pole switch. It appears to be a single throw single pole on off switch with illumination? A 5 pole would be able to switch five different appliances.
