I’m assuming the term PCV valve is the same as a crankcase. I have 2 in my 05 and one is clearly leaking. Is it possible I only need to replace the grommet on the one? Or should I buy the valve as well?
I would definitely change the grommets as well. One is just a breather and the other is the PCV. I don't recall which is which. Check out this thread....

Yeah I read that thread but no one differentiated the two. The one dude left the part numbers (cheers). One is just a breather and the other is the PCV. I don't recall which is which. Check out this thread....

Cheers for Distinguishing what part number goes with what valve. The odds of removing these in one piece are low. This job is best done when replacing the valve cover gasket.

I was under the impression that neither of these were actually PCV valves, but just breather tubes. Can somebody confirm? Just curious.
How hard/long to replace the gasket? The odds of removing these in one piece are low. This job is best done when replacing the valve cover gasket.

In my picture the leaky one is 53030495? I’m assuming the front means closest to the bumper. Or am I backwardsThe odds of removing these in one piece are low. This job is best done when replacing the valve cover gasket.

Exactly. The rear isn't a "valve" at all, its just an elbow with a restrictor in it, .060" if I remember correctly. They get brittle with age, but a lot of auto parts stores only sell the front - completely open - elbow for both front and rear. You'll have a very high idle if you install open elbows in both locations. The dealership did this to mine just before I bought it...The 4.0L uses a CCV system, not a PCV system. That front hose you see should go into the valve cover, not the crankcase. The front hose is the clean air inlet. The skinny little tube coming out of the rear of the valve cover is the outlet. It goes to the intake manifold, and that's what creates the suction to circulate air through the crankcase to remove fumes.
Grommets and valve. The valve will be brittle anyway from engine heat. It will cone out in pieces.I’m assuming the term PCV valve is the same as a crankcase.
